Privacy policy
We handle health information. This explains what we collect, why, where it is stored, and what you can do about it.

Who we are
HMR Consultants Australia provides Home Medicines Reviews on referral from general practitioners. As a health service provider we are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles regardless of turnover, and by the Health Records and Information Privacy Act 2002 (NSW) for NSW patients.
Patient identification searches
Where we help a practice identify patients who may be eligible, the search runs inside that practice's own software, against their own records. We do not receive, store or hold a copy of the practice's patient list at any point. The practice remains the holder of that information.
Where we contact patients on a practice's behalf, we do so as that practice's service provider under a written data-handling agreement, with the practice identified as sender. Every message carries a working opt-out. We maintain the opt-out register and honour opt-outs permanently. Nothing is sent until a GP at the practice has reviewed and approved the list.
What we collect
- Your name, date of birth, address and contact details
- Your Medicare or DVA number
- Your medicines, allergies, medical history and relevant pathology
- Your GP's details and the referral they sent
- Notes made during the review
We collect this from your GP's referral and from you during the visit.
Why we collect it
To conduct the review, write the report to your GP, and claim the service from the Pharmacy Programs Administrator. We do not use your health information for marketing and we do not sell it.
Where it is stored
In our practice management system on Australian servers, handled by Australian-based personnel. No overseas contractor, administrative service or support staff has access.
This website collects no health information. The referral and booking forms are hosted by our practice management system. Data entered there goes directly into the patient record and never touches this website.
Who we share it with
- Your referring GP: the report and any follow-up
- The Pharmacy Programs Administrator: the minimum required to claim the service
- Others you ask us to: a specialist, carer or family member, with your consent
We will not disclose your information to anyone else unless required by law.
How long we keep it
Health records are retained for a minimum of 7 years from the date of last service, and for patients under 18, until they turn 25. This is consistent with NSW health record requirements.
Cookies and tracking
This website sets no cookies, runs no advertising trackers and loads no third-party fonts or scripts. The referral and booking forms are hosted by our practice management system, which sets its own cookies when used.
Accessing or correcting your information
Email simon@hmrconsultants.com.au or ring 1800 701 520. We respond within 30 days.
Data breaches
We maintain a response plan under the Notifiable Data Breaches scheme. If a breach were likely to cause you serious harm we would notify you and the Office of the Australian Information Commissioner.
