Home Medicines Review in Sydney
We visit patients at home right across Greater Sydney, with most of our work in the south west and the south. The visit is free, it takes about an hour, and the only thing you need is a referral from your GP.

Greater Sydney, weighted to the south west and the south. If you are in the inner city or the northern beaches we will still take the referral, we will just be honest with you about how soon we can get there.
Suburbs include Liverpool, Bankstown, Campbelltown, Fairfield, Camden, Cabramatta, Ingleburn, Minto, Narellan, Casula, Moorebank, Revesby, Padstow, Panania, Chipping Norton, Prestons, Edmondson Park, Leppington, Oran Park, Gregory Hills, Wetherill Park, Bonnyrigg, Green Valley, Miller, Sutherland, Menai, Engadine and the surrounding suburbs.

Why this comes up here.
Sydney is not one market. A patient discharged from Liverpool Hospital on a Friday afternoon and a patient who has quietly accumulated 11 medicines over 15 years with the same GP need the same visit, but they arrive from completely different directions. We take both.

They assess you and refer under MBS item 900. It costs them nothing and it costs you nothing.

To arrange a time that suits, at your home. You do not have to go anywhere.
The visit
About an hour at the kitchen table, going through everything you take, including what you buy yourself.
The report
Back to your GP within 7 days, then a plan agreed with you. Nothing changes without your GP.
Questions
Is the visit really free?
Yes. There is no charge to you at any point, no gap and nothing to claim back. You need a current Medicare or DVA card. More on what it costs.
How do I arrange one?
Ask your GP for a Home Medicines Review at your next appointment. They refer under MBS item 900 and we take it from there. You cannot arrange one directly and neither can a pharmacist.
How soon can you come?
We ring to arrange a time as soon as the referral arrives. Most visits happen inside 2 weeks. The program requires the visit to happen within 90 days of the referral date, and we track that so nothing lapses.
What happens after the visit?
A written report goes to your GP within 7 days. Your GP then discusses it with the pharmacist and agrees a medication management plan with you.
