Campbelltown Hospital is the main acute service for Macarthur, and a discharge from it is one of the most common reasons a review is worth doing. Medicines get changed during an admission and the discharge summary does not always match what is still sitting in the kitchen at home. That gap is the single most common thing we find.

How do I arrange one?
Ask your GP for a Home Medicines Review at your next appointment. They refer under MBS item 900 and we take it from there. You cannot arrange one directly and neither can a pharmacist.
How soon can you come?
We ring to arrange a time as soon as the referral arrives. Most visits happen inside 2 weeks. The program requires the visit to happen within 90 days of the referral date, and we track that so nothing lapses.
What happens after the visit?
A written report goes to your GP within 7 days. Your GP then discusses it with the pharmacist and agrees a medication management plan with you.
