Why this comes up here.
A practical note about the area. Camden Hospital has an emergency department but it is a self-presenting service that does not take ambulance arrivals, so Campbelltown Hospital is where most Macarthur patients are actually admitted and discharged from. If you are referring after a hospital stay, that is usually the discharge summary to look at.

How do I arrange one?
Ask your GP for a Home Medicines Review at your next appointment. They refer under MBS item 900 and we take it from there. You cannot arrange one directly and neither can a pharmacist.
How soon can you come?
We ring to arrange a time as soon as the referral arrives. Most visits happen inside 2 weeks. The program requires the visit to happen within 90 days of the referral date, and we track that so nothing lapses.
What happens after the visit?
A written report goes to your GP within 7 days. Your GP then discusses it with the pharmacist and agrees a medication management plan with you.
