One thing that is specific to the Illawarra. A lot of people here have retired to the coast from Sydney, which means their specialists, their old GP and their memory of who started what are often still 80 kilometres away. Nobody local has ever seen the full history. That is precisely the situation a home visit is built for.

Is the visit really free?
Yes. There is no charge to you at any point, no gap and nothing to claim back. You need a current Medicare or DVA card. More on what it costs.
How do I arrange one?
Ask your GP for a Home Medicines Review at your next appointment. They refer under MBS item 900 and we take it from there. You cannot arrange one directly and neither can a pharmacist.
How soon can you come?
We ring to arrange a time as soon as the referral arrives. Most visits happen inside 2 weeks. The program requires the visit to happen within 90 days of the referral date, and we track that so nothing lapses.
What happens after the visit?
A written report goes to your GP within 7 days. Your GP then discusses it with the pharmacist and agrees a medication management plan with you.
