Where we visit

Reviews are conducted in the patient's own home, across:

  • Greater Sydney
  • South Western Sydney and Macarthur
  • Illawarra and Shoalhaven
  • Southern Highlands

Outside these areas, ring and ask. We can often help or point you to someone who can.

Please don't email patient health information. Email is not a secure channel. Use secure messaging or the referral form, or ring us and we'll find a safe way to receive it.

For patients, family and carers

Ask us to call you back.

You cannot refer yourself for a Home Medicines Review, and neither can a pharmacist. It has to come from your GP. What we can do is explain how it works, tell you honestly whether it is likely to be worth it, and give you the words to use at your next appointment. That costs nothing and commits you to nothing.

Please do not list your medicines here. We will go through all of that properly at the visit, or on the phone.

Before you fill that in

  • It is free. There is no charge for the review, the visit, the report or the follow-ups. Nothing to pay and nothing to claim back.
  • Your GP has to refer you. We can tell you exactly what to ask for. Most GPs know it immediately as a Home Medicines Review, or MBS item 900.
  • We do not need your medicines list now. Please leave it off the form. That conversation belongs at the kitchen table, not in a web form.
  • What you send is encrypted before it is stored, kept in Australia, and read only by us.
  • Nothing is shared with your GP without your say so. Ticking the box lets us ring your practice. If you would rather we did not, leave it unticked and we will only speak to you.
